In week 2, we were allocated a short period of time in our groups to change a list of titles into a simple data collection form - focussing on the components rather than the aesthetics. In our group we decided to include as many drop down lists as possible (such as on the title and date of birth fields) this allows users to only choice from a specific list of options, reducing errors within the form and increasing the speed of the form being filled in. Another component we considered is adding the possibility to search for your address or GP address via the postcode - again to reduce errors and increase efficiency. One last addition we included was a tick-box to allow yes or no answers to be quickly answered, like in the "Were you born abroad?" field.

This task allowed my group to think logically about how we could make this process as quick as possible, as with this form we want users to have to spend as little time as possible as research shows the longer forms are the less accurate responses are as they move through. This was a process we can trough into each of our projects to help us think logically about the efficiency of our products.
